The role involves investigating, assessing, and planning interventions to assist patients with social, emotional, economic, and environmental challenges. This includes determining and arranging necessary internal and external services, participating in discharge planning, and coordinating care with multidisciplinary teams.
The position requires interviewing patients and their representatives to determine needs, providing necessary services or referrals, and performing chart reviews to monitor service delivery and compliance. The role is classified as non-exempt and requires 36 scheduled weekly hours.
Provides comprehensive care management services to inpatient behavioral health patients by assessing needs, coordinating services, supporting safe transitions of care, and collaborating closely with a multidisciplinary team to promote positive patient outcomes.
Position: Inpatient Behavioral Health Case Manager Location: Inpatient Behavioral Health / Psychiatric Unit Schedule: 36 hours per week Shift: Day shift, 4x9's (typically 7:30 am–5:00 pm); no weekend requirement, flexibility based on patient census What You'll Do As an Inpatient Behavioral Health Case Manager on our close-knit psychiatric care team, you’ll play a vital role in coordinating patient-centered care while ensuring no one carries a patient alone — collaboration and communication are at the heart of how we work. Clinical Responsibilities Interviews and assesses patients and/or patients’ families, caregivers, and/or legal representatives to identify clinical, psychosocial, and resource needs Determines, prioritizes, provides, and/or arranges for appropriate internal and external services and interventions Consults regularly with multidisciplinary teams to evaluate patient status, treatment progress, and appropriateness of medical and behavioral health care across care settings Performs ongoing chart review to identify actual or potential issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement Care Coordination & Discharge Planning Actively participates in discharge planning, including identifying and coordinating appropriate post-discharge resources Provides discharge education and resource referrals to patients and families to support safe transitions of care Coordinates care for an inpatient psychiatric population, including patients undergoing alcohol and opioid detox What We're Looking For Experience in behavioral health, care management, or related clinical setting Must hold a Maine State license as a Licensed Social Worker (LSW OR LSX), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Registered Nurse (RN) Strong assessment, communication, and care coordination skills Ability to work collaboratively within a multidisciplinary team A patient-centered, compassionate approach to care management in an inpatient setting Why This Role Stands Out Day shift schedule with predictable hours and flexibility as census allows Inpatient behavioral health setting with a mix of psychiatric care, alcohol detox, and opioid detox Highly supportive, close-knit team where communication is strong and no one manages patients alone Meaningful work that directly supports patient safety, recovery, and successful transitions of care Why MaineGeneral?
