The Physical Therapist will evaluate patients' physical abilities and functional limitations in home health settings, developing and implementing individualized treatment plans to restore function and alleviate pain. Responsibilities also include educating patients and caregivers, monitoring progress, documenting care, and collaborating with the interdisciplinary team.
Candidates must possess an active Physical Therapist license in Texas and professional liability insurance specific to Texas Physical Therapy. A graduate degree or higher in Physical Therapy is required, with home health or rehabilitative experience being preferred.
