The Nurse Care Manager will serve adult clients with severe mental illness, substance use disorder, or co-occurring disorders, providing a mix of office and field-based services. Key duties include medication reconciliation, coordinating care with primary providers, offering nursing assessments, and managing follow-up from hospital visits.
Candidates must have completed a Registered Nursing (RN) Program and hold a current RN license in the State of Oklahoma. The role involves working within the Certified Community Behavioral Health Clinic (CCBHC) model, focusing on team-based, whole-person care for complex behavioral health challenges.
Requires completion of Registered Nursing (RN) Program, current licenses as an RN in the State of Oklahoma.
