The Case Manager provides mental health rehabilitation services and support to adults and families aiming to decrease hospitalization and crisis episodes while increasing community functioning to meet recovery goals. Responsibilities include undertaking primary case management duties, conducting various counseling sessions, maintaining client service records, and coordinating services from community resources.
Candidates must possess a Bachelor’s degree in Psychology, Counseling, Social Work, or a related field, or equivalent experience, and must qualify as a Mental Health Professional (MHP) per Illinois Department of Mental Health Rule 132. Required skills include demonstrated proficiency in case management, counseling, accurate documentation, and competence with common computer applications, along with personal transportation and a valid driver's license.

Provides mental health rehabilitation services and supports adults and families to decrease hospitalization and crisis episodes and to increase community functioning in order for the client to achieve rehabilitative, resiliency and recovery goals. Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of mental health services to clients and their families and facilitate the issues of recovery into their daily living situations.
• Monday-Friday 8:30am-5:00pm
Hybrid work model, this role combines work from home, time in the office, and fieldwork across multiple counties in Northwest Illinois, in the greater Rockford area
