The Nurse Navigator provides extensive patient education regarding illness, management, and treatment, while facilitating care coordination for Infectious Disease patients. This role also involves providing necessary in-clinic support to providers as required.
Candidates must possess a Bachelor's degree in Nursing or higher, along with current D.C. Registered Nurse licensure or evidence of application. A minimum of two years of experience as a Registered Nurse is mandatory for this position.
Under general administrative direction, provides a significant level of patient education related to illness and planned management and treatment and facilitates coordination of care for INFECTIOUS DISEASES patients. In addition, the Nurse Navigator provides in-clinic support to providers, as needed.
A minimum of 2 years of experience as a Registered Nurse is required.
