The Social Worker 2 collaborates with the healthcare team, patient, and family to coordinate timely psychosocial assessments and formulate patient-centered discharge plans. Responsibilities include providing psychosocial assessment and intervention, developing comprehensive plans based on identified needs, and linking patients to community and internal resources.
A Master's Degree from an accredited School of Social Work is required, along with proficiency in computer skills, theoretical/clinical knowledge of healthcare delivery, multitasking, prioritization, autonomy, and effective communication. Hospital/Health Agency experience is preferred, and a Licensed Social Worker (LSW) certification is mandatory.

Previous work in a hospital setting is strongly preferred. This PRN position will travel between Lake West and TriPoint Medical Centers. The Social Worker will be required to work in any unit of the hospital as needed, to perform psychosocial assessments, provide support to patients and families, and assist with discharge planning. It is needed to cover weekdays and weekends.
