The Nurse Practitioner provides comprehensive hospice services, including evaluating patient health, developing individualized Plans of Care, prescribing treatments, and conducting face-to-face assessments for hospice eligibility under physician supervision. This role involves extensive collaboration with the interdisciplinary team, attending physicians, and families to ensure effective symptom control and continuity of care for terminally ill patients.
Candidates must possess a current Nurse Practitioner (NP) or Advanced Practice Registered Nurses (APRN) license and a degree from an accredited nursing and NP program, along with a minimum of two years of RN experience. A valid driver's license, access to an insured vehicle, and the ability to pass background checks are also mandatory requirements.
