The LPN performs delegated nursing and medical functions, including data collection and care delivery for an assigned group of patients, operating under the direction of a Registered Nurse or authorized practitioner. Responsibilities involve following the patient's plan of care, prioritizing work based on acuity, and performing defined technical procedures.
Candidates must be graduates of an accredited LPN program and possess a current LPN License along with Basic Life Support Certification. Preferred qualifications include Ambulatory Care Experience and at least one year of clinical experience.
Follows the patient’s plan of care. Prioritizes work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner. Performs high and low risk technical procedures, including medications, defined by the clinical setting. Recognizes when different approaches may be needed based on age or cultural differences. Implements assigned aspects of the patient teaching plan. Documents care in the medical record. Effectively communicates compliance and concerns to delegating RN or physician/authorized practitioner. Takes action to assure continuity of care for all assigned patients. Adheres to all relevant policies, procedures, and nursing practice guidelines. Responds appropriately to emergent situations. Documents care in the medical record. Communicates verbal and written information in a clear, concise, accurate, and timely manner. Use communication approaches appropriate to patient/family needs. Required Qualifications Graduate of an accredited LPN program is required. Current LPN License Basic Life Support Certification is required.
Ambulatory Care Experience is preferred. At least 1 year of Clinical Experience is preferred.
