The Medical Social Worker provides social work services in accordance with the physician’s plan of treatment and assists clients and families in coping with daily living problems. They develop care plans, participate in discharge planning, and coordinate with other health professionals to ensure continuity of care.
Candidates must have a master's degree in social work and be licensed in the state of practice. A minimum of one year of experience in social work, preferably in a home care setting, is required.

Pay Range $65,000—$70,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
