The Wellness Coach provides case management and supportive counseling to agency consumers, assisting them with accessing community services and crisis intervention. They also maintain documentation, conduct home and hospital visits, and collaborate with the social service team to develop community integration activities.
A Bachelor's degree is required for this position. Candidates should have experience with individuals with mental illness, substance use issues, or homelessness, along with proficiency in Microsoft Office programs.
