The Children's Behavioral Health Home Coordinator will coordinate care and support services for children with behavioral health needs, working closely with families and treatment providers. They will lead care planning, advocate for clients, and ensure families receive necessary services.
A bachelor's degree in a relevant field is required, or a degree in an unrelated field with relevant experience. Candidates must also complete the CANS assessment tool and possess a valid driver's license.
Description Are you a compassionate and organized professional with a passion for helping children and families thrive? AMHC is seeking a Children’s Community Case Manager to join our Madawaska team on a full-time basis.
In this key role, you’ll coordinate care and support services for children with behavioral health needs, working closely with families, treatment providers, and community partners. You’ll lead care planning, advocate for clients, and ensure families get the services they need—all within a supportive, collaborative environment.
Build strong, trusting relationships with children and families, even in challenging engagement situations. Develop and update individualized treatment plans in collaboration with families and providers. Coordinate and monitor services in line with treatment goals. Facilitate quarterly treatment team meetings and lead multidisciplinary collaboration. Advocate for families in schools, social services, and healthcare settings. Educate families about available behavioral health resources and support systems. Respond to crises and support the development of crisis and safety plans. Maintain timely, accurate documentation and ensure compliance with program and regulatory standards.

This is a non-exempt full-time position (40 hours/week).

The position requires a bachelor’s degree with a specialization in psychology, mental health, human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field. OR have a bachelor’s degree from an accredited four (4) year educational institution in an unrelated field and at least one year of full-time equivalent relevant human services experience. Must complete the Child and Adolescent Needs and Strengths (CANS) assessment tool and pass the exam annually. Must possess a valid driver’s license and reliable transportation.
