The Service Coordinator provides comprehensive case management for individuals aged 14 to 23, coordinating medical, behavioral, and educational services. They are responsible for developing Individual Program Plans (IPP) and facilitating collaboration between families, vendors, and community resources.
Candidates must hold a bachelor's degree in a related field such as social work or psychology, with at least one year of experience working with individuals with developmental disabilities. A valid driver's license and reliable transportation are required for community-based meetings.
San Gabriel/Pomona Regional Center (SG/PRC) is a private, nonprofit agency contracted with the California Department of Developmental Services (DDS) to provide services and supports to individuals with intellectual and developmental disabilities. We proudly serve 30 cities across the Foothill, Pomona, and El Monte Health Districts within Los Angeles County. Our services span the full lifespan—from infants and toddlers, school-age children, and transition-age youth to adults and aging adults. Reporting to the Manager of Transition Services IV, the Service Coordinator provides comprehensive and compassionate case management to individuals ages 14 to 23 and their families. This role offers the opportunity to make a meaningful difference working collaboratively with families and community resources to ensure the individuals we serve receive high-quality service support and thrive.
