The coordinator performs health and functional assessments to develop and monitor individualized care plans for older adults and individuals with disabilities. They provide regular contact and home visits to ensure members progress toward their goals in the least restrictive environment.
Candidates should have at least two years of experience in human services, preferably with the elderly, and a bachelor's degree in social work or a related field. Proficiency in Microsoft Office and bilingual skills are highly preferred.
