The Mobile Case Manager provides community-based support and aftercare services to youth exiting shelter to ensure continuity of care. This role involves conducting proactive outreach, maintaining shelter integration, and documenting case activities to support youth stability.
Candidates must have a bachelor's degree in a human services field and two years of related case management experience, or equivalent experience. A valid driver's license, reliable transportation, and the ability to pass a background screening are required.

Sign on Bonus of $600 (Half paid out at 3 months and the other half at 12 months).
$24.04 – 26.44/hour; $50,000 - $55,000/annually
180 Degrees’ youth shelters serve adolescents ages 12–19 who are seeking safety, stability, and support during critical moments in their lives. Youth may enter shelter through county placements, or because they are experiencing homelessness, running away from home, family conflict, exploitation, or safety concerns. Many youth have experienced trauma, instability, or disrupted relationships and come to us at a time when trust, consistency, and dignity matter deeply.
The Mobile Case Manager (MCM) provides prevention-focused, community-based support that bridges shelter services and the broader community. This role ensures continuity of care for youth exiting shelter through structured aftercare services while maintaining a consistent outreach presence in spaces where youth naturally gather.
As part of 180 Degrees’ continuity-of-care model, the MCM supports youth across transitions and collaborates closely with shelter staff and other Mobile Case Managers across the organization to maintain alignment as youth move between programs and communities. While the position operates with a high level of independence in the field, it remains grounded in supervision, teamwork, and consistent connection to shelter operations.
This position does not have direct reports. This position is 40 hours per week, onsite.
