The LCSW provides comprehensive case management services, including assessments and crisis intervention for clients and their families. They develop treatment plans and support the medical treatment plan while ensuring effective communication with the clinical care team.
Candidates must have a Master's degree in Social Work or a related field and an active NC LCSW license. A minimum of 4 years of professional experience in a community healthcare environment is required.

Under general supervision, the LCSW provides comprehensive case management services for all clients and their family members. Responsibilities include initial and on-going assessment of bio-psychosocial factors, healthcare factors and educations needs of these clients with a focus on enhancing our client's capacity for self-support and self-sufficiency.
