The Patient Clinical Navigator RN monitors interdisciplinary inpatient programs and serves as a primary resource for patients navigating their healthcare experience. They facilitate communication between medical staff and families, provide case management, and ensure compliance with hospital policies and safety standards.
Candidates must hold a valid Indiana Registered Nurse license, BLS, and ACLS certifications. A minimum of two years of current inpatient medical hospital experience and one year of navigation or collaborative health-related experience is required.
The Patient Navigator is a Registered Nurse who monitors the interdisciplinary components of inpatient programs to ensure consistency with the overall strategic plan of the hospital. This position serves as the primary resource for patients and their health care partners in the navigation of the health care experience for medical health issues.
