The Physical Therapist will evaluate patients, develop individualized care plans, and deliver skilled therapy services within a skilled nursing facility. They will collaborate with interdisciplinary teams to track functional progress and support safe discharge planning.
Candidates must be graduates of an accredited Physical Therapy program and hold an active state PT license. Strong clinical reasoning, documentation skills, and a commitment to patient-centered care are required.
Graduate of an accredited Physical Therapy program (PT/DPT). Active state PT license (or ability to obtain prior to assignment). Strong clinical reasoning, communication, and documentation skills. A positive, flexible mindset and commitment to excellent patient care.
