Position Overview

• Inabia is looking for a Physical Therapist to join our team in a Skilled Nursing Facility (SNF) setting.

• In this role, you’ll evaluate patients, build individualized plans of care, and deliver skilled interventions that help people move better and live better.
• You’ll partner closely with the interdisciplinary team and educate patients/caregivers to support safe progress and smooth transitions of care..
• Key Responsibilities Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
• Provide daily treatments, track functional progress, and support discharge planning.
• Collaborate closely with therapy, nursing, and interdisciplinary teams.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in skilled nursing rehab programming focused on functional recovery and safe transitions.
• Contribute to a team-based approach that prioritizes patient outcomes and quality care.

Qualifications

Graduate of an accredited Physical Therapy program (PT/DPT). Active state PT license (or ability to obtain prior to assignment). Strong clinical reasoning, communication, and documentation skills. A positive, flexible mindset and commitment to excellent patient care.
