The physical therapist will perform outpatient physical therapy in a home setting, including patient evaluation, treatment, and case management. They are responsible for accurate documentation, interpreting diagnostic data, and ensuring medical orders are processed correctly.
Candidates must hold current state licensure or eligibility for licensure as a Physical Therapist. Certification in BLS is required or must be obtained within the first 90 days of employment.

Outpatient at Home is seeking a PRN skilled outpatient physical therapists in the North Texas area. The clinician selected for this position would be performing OUTPATIENT physical therapy in the home setting. Responsibilities include evaluation, treatment, case management, and patient assessment of patients with orthopedic injuries or higher level neurological conditions. This individual would require high level autonomy, time management, and communication skills. Prior outpatient and/or home health experience is preferred. Coverage territory: Rockwall County Initial and ongoing systematic patient assessment Patient Case Load Average: 6 patients a day Timely and accurate documentation using Therapy Source Interpreting assessment/diagnostic data including medical notes and surgery reports Ensuring medical orders are transcribed and processed accurately Promoting continuous quality improvement Instructing and counseling patients/families as needed
