• The Home Health PRN Field LVN/LPN plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective management of health conditions.
• This position requires assessing patient health status, administering medications, and providing wound care, while collaborating closely with physicians, therapists, and family members.
• The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being.
• This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance.
• Ultimately, the Home Health Field LVN/LPN contributes to enhancing patient outcomes by providing personalized, attentive care in a home setting, supporting recovery and chronic disease management outside of traditional clinical environments.

As a Home Health LVN, you will

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Promote Agency philosophy and administrative policies to ensure quality of care.

• HomeCare HomeBase experience preferred
