The provider will perform comprehensive clinical assessments and manage acute and chronic conditions for residents in long-term care facilities. They are responsible for weekend rounding, developing individualized care plans, and collaborating with interdisciplinary teams to reduce hospitalizations.
Candidates must hold an advanced degree such as an MSN, DNP, or MPAS, along with active state licensure and board certification. A minimum of 2 years of clinical experience in long-term care or geriatric settings is preferred.

The provider will be responsible for rounding in person on Saturday & Sunday to address acute issues and take call for the facilities during the weekend. Typical Coverage hours are 5pm Friday until 8am Monday. MSN, DNP, MPAS, DMSc, or equivalent advanced degree. Active state licensure and board certification (AANP/ANCC/NCCPA). DEA and BLS certification. Strong interpersonal communication and knowledge of clinical practice standards. It’s A Plus If You Have 2+ years of clinical experience. Long Term Care, Skilled Nursing Facility, or geriatric experience.
