Job Summary We are seeking compassionate and dependable Certified Nursing Assistants (CNAs) / Home Service Workers to provide high-quality, person-centered care to individuals in their homes and community settings, including assisted living and long-term care environments. In this role, you will support clients with daily living activities while promoting dignity, independence, and overall well-being. This position reports to the Homecare Supervisor. This position offers the opportunity to make a meaningful difference in people’s lives through respectful, attentive, and reliable caregiving.

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, eating, toileting, and mobility Provide medication reminders in accordance with care plans and HIPAA guidelines Complete accurate daily documentation of services provided Support clients with memory care needs, including dementia and Alzheimer’s care Prepare nutritious meals based on individual dietary needs and assist with feeding when necessary Perform light housekeeping and laundry to maintain a safe and clean living environment Observe, document, and report changes in client condition, behavior, or needs to the Home Care Supervisor Safely operate assistive devices, including Hoyer lifts and transfer equipment Provide emotional support, companionship, and social engagement Communicate effectively with clients and family members Follow safety protocols, including CPR and First Aid procedures Complete incident reports and participate in investigations as required
Must be at least 18 years of age High school diploma or GED required Successful completion of a state-approved CNA or Personal Care Assistant program, or comparable experience Ability to lift up to 50 pounds Valid CPR and First Aid certification (or willingness to obtain) TB test and physical as required Successful completion of a background check Valid driver’s license Valid work authorization, if applicable Reliable transportation (personal vehicle with insurance or other dependable means) Minimum of 1 year of experience in home and community-based care, assisted living, nursing homes, or hospice care
Experience working with individuals with physical or developmental disabilities, autism, dementia, or Alzheimer’s disease Knowledge of client observation and reporting techniques Familiarity with HIPAA and confidentiality standards Experience using Hoyer lifts and safe transfer techniques Ability to read, write, speak, and understand English as required for the role; bilingual Spanish is a plus Strong dependability and reliability Ability to follow care plans, procedures, and assignments Friendly, caring, and compassionate demeanor Patient-focused mindset with strong communication and teamwork skills Disclaimer This job description is intended to describe the general nature and level of work being performed.
