The Ambulatory Clinical Pharmacist identifies and resolves medication-related problems to optimize therapeutic outcomes for patients with chronic conditions. They conduct comprehensive assessments and provide individualized counseling while working on-site with interdisciplinary teams.
Candidates must hold a Doctor of Pharmacy (PharmD) degree and an active Texas pharmacist license. A minimum of 3 years of clinical pharmacy experience is required, with PGY1/PGY2 residency highly preferred.
