The Bereavement Coordinator assesses hospice families to identify bereavement needs and provides ongoing support through documentation and community referrals. They also collaborate with the interdisciplinary team to coordinate care plans and monitor volunteers within the bereavement program.
Candidates must be a high school graduate with a preference for a degree or license in counseling. Experience in bereavement and grief counseling, particularly in hospice or palliative care, is required along with a valid driver's license and insurance.

High School Graduate, Degree/License in counseling preferred Experience in bereavement and grief counseling, preferably in hospice and palliative care setting; grief support counseling Working knowledge of community resources Demonstrates good communication, interpersonal, negotiation, and public relations skills Experience with EMR software is a plus Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance
