Provide direct nursing care and coordinate multidisciplinary services to develop and implement patient plans of care. Document clinical assessments and provide essential education and support to patients' families and friends.
Requires a graduate degree from an accredited school of nursing with a current Texas RN license and BLS certification. Experience in hospice or hospital settings and proficiency with clinical ERP systems are preferred.
