The Case Manager coordinates services and provides supportive counseling for chronically mentally ill adults to assist with daily living and treatment plan goals. They also perform community outreach, maintain clinical records, and ensure effective communication between all parties involved in the client's care.
The position prefers a Bachelor’s degree in Social Work, Psychology, or a related Human Services field, though 1-3 years of equivalent experience will be considered. Candidates must possess a valid driver's license and basic computer skills including typing and email usage.

Under the direction of the Outpatient/ TCM Community Service Manager, the Case Manager implements, supports, and helps coordinate the services for chronically mentally ill adults living in the community in conjunction with their individual treatment plans.
Provides case management, service coordination, supportive counseling, and skill building for assigned clients. Supports daily living skills, shopping, budgeting, and coordinates services for meeting housing needs for clients. Provides assistance in getting income support and entitlement. Helps to foster peer supports. Facilitates growth of community and family support. Facilitates assistance to meet necessary mental and physical health needs. Participates in all aspects of TCM.
If or as assigned to provide Substance Use Services, or within related SUD grants, the incumbent will show competencies in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories, in order to provide effective care.
Provides outreach services to open clients of the community who are in need of our services. Develops systems to provide services to area clients. Provides protection and advocacy of client’s rights whenever possible. Provides clients with referral information and assists with transportation as needed. Provides support to group home clients as needed.
Completes various clinical and administrative forms for billing, information, and clinical records. Completes all case management documentation within 24 hours and finalized within 3 business days and in a professional manner. Attends all staff meetings, agency meetings, in-services, and clinical trainings as instructed by supervisor. Maintains confidentiality on all cases at all times. Follows goals of the program and policies of United Counseling Service.
