The Community Health Worker serves as a liaison between health services and the community, providing health navigation, coaching, and patient education via telephone and in-person meetings. They are responsible for documenting interactions, collecting grant-related data, and building relationships with community-based organizations to support patients with substance use disorders.
A high school diploma or GED is required, with a preference for graduates of a community health worker training program and certification. Candidates must possess strong communication and organizational skills, the ability to work with diverse populations, and proficiency in Microsoft Office.
