The MDS Coordinator leads the Resident Assessment Instrument (RAI) process to ensure accurate, timely, and compliant MDS assessments and care plans. This role collaborates with the interdisciplinary team to support clinical documentation, reimbursement accuracy, and quality performance.
Candidates must be a Registered Nurse in Colorado with at least two years of direct MDS experience in a skilled nursing facility. Professional certifications such as RAC-CT and RAC-CTA are required or must be obtained within specified timeframes.

This role supports regulatory compliance, reimbursement accuracy, and quality performance by working collaboratively with the interdisciplinary team, analyzing documentation and data, and driving continuous improvement in assessment and care planning practices. The MDS Coordinator is expected to fully utilize available systems and resources to support optimal clinical and financial outcomes aligned with organizational priorities.
