Patient Navigator (Foster Care) - 40 hours, Days

Preferred

• 1. Bachelor’s Degree in Social Work, Human Services or Counseling.
• Experience/Skills:
• Required: 1. Two years of social work, human service or related professional experience, including demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, and ability to work in a multi-disciplinary team. 2. Ability to communicate clearly and concisely in both oral and written form. 3. Personal experience with DCF, Foster Care and navigating through the processes to regain custody.

Preferred

• 1. Bilingual in English and Spanish. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. *Seeking a Patient Navigator who can be a peer to our patient community when going through the process of regaining custody of children in the foster care system. The ideal candidate has experience with having a child(ren) removed but navigated through the process and regained/maintained custody.
