The Social Worker performs psychosocial assessments of patients and families to develop and implement supportive counseling and intervention plans. They also provide psychosocial education and consultation to the interdisciplinary group to ensure comprehensive patient care.
The role requires a professional social worker capable of working within an interdisciplinary team to support hospice patients. Candidates must demonstrate strong communication skills and a commitment to professional growth and compassionate care.
