The Palliative Care Nurse Practitioner will provide high-quality palliative care services across home and facility settings while managing complex symptoms and leading goals-of-care discussions. Additionally, the role involves conducting hospice face-to-face encounters, ensuring regulatory compliance, and helping to build workflows for the palliative care program.
Candidates must hold an active New Jersey APN license and board certification (FNP, AGNP, or equivalent). A minimum of 2–3 years of hospice or palliative care experience is required, along with strong knowledge of E/M coding and Medicare billing requirements.

We are a growing hospice organization in New Jersey expanding services to include a palliative care program. Our mission is to provide compassionate, patient-centered care to individuals with serious illness while supporting families and caregivers. We are seeking an experienced Nurse Practitioner to help develop and drive the plan of care for palliative care for our existing hospice program.
The Palliative Care Nurse Practitioner (NP) will provide high-quality palliative care services across home and facility settings while supporting hospice operations. This role includes conducting hospice face-to-face (F2F) encounters in compliance with CMS and New Jersey regulations, ensuring accurate documentation, and supporting appropriate billing and coding practices. The NP will play a key role in building workflows and clinical standards for the palliative care program.
