The Advanced Practice Provider will conduct daily patient rounding via telehealth and perform in-person assessments at patient homes when necessary. They are responsible for managing admissions, discharges, and consultations while collaborating with multidisciplinary teams to ensure high-quality patient care.
Candidates must be nationally board-certified as an Acute Care NP, Adult Gerontology Acute Care NP, or a Physician Assistant. A safe driving record and the ability to travel between TGH facilities and patient homes are required.
Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant. DEA required or obtained after the orientation period. Inpatient Hospital care and Internal Medicine preferred. Safe driving record required to operate hospital at home vehicles. Demonstrates excellent communication, critical thinking, and leadership skills. Ability to work effectively in a team. Commitment to ongoing professional development and process improvement.
