Cottage Hospital is seeking a compassionate and skilled Social Worker to support patients through comprehensive psychosocial care. In this role, you will conduct thorough assessments to identify patient needs, establish diagnoses, and develop individualized treatment plans that support overall well-being.
You will serve as an active member of the interdisciplinary care team, providing emotional support and mental health counseling to both individuals and groups. You will manage referrals to outside providers and agencies, helping patients navigate care options and ensuring continuity of treatment.
