AHCH currently has an exciting opportunity for a Community Support Worker (CSW) who will work primarily in our new Medical Respite facility at the Gateway Center located at 5400 Gibson SE in Albuquerque. This position will also, on occasion, work at our 1217 1st St location downtown and will be dual certified as a Community Health Worker (CHW) to best meet the needs of the Respite clients.
Our ideal candidate is someone with strong critical thinking skills, a client-centered mindset, and the ability to work well with diverse populations. Though there are no home visits associated with this CSW position, the candidate must be comfortable with transporting clients in our company vehicles. The caseload will vary dependent on need and can include up to 30 patients. Candidates must be able to complete documentation of encounters within 72 hours of the visit date.

Assess individual client needs in conjunction with medical team at Medical Respite Create individualized care plans in conjunction with a medical provider and with direct input from the client and in accordance with Comprehensive Community Support Services (CCSS) and Medicaid requirements Work in an integrated manner with other AHCH programs to optimize client care Refer client to appropriate programs and agencies for coordination of wrap-around services in the community Develop and deliver discharge plan with medical and/or behavioral health Participate in client chart audits for peer review Maintain dual certification as Community Support Worker (CSW) and Community Health Worker (CHW) Perform duties in a trauma informed and client centered manner Participate in planning, reporting, and evaluating program components to meet program guidelines and national standards Ensure accurate and appropriate documentation of all client contacts in Electronic Health Record (EHR) within timeframe required by AHCH procedure and billing standards (72 hours from date of service) Attend program-specific meetings and training including clinical supervision with Behavioral Health and Primary Care Attend organizational meetings and training
Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop
