Provide social work services within a community health setting, focusing on patient navigation and access to care. Support geriatric populations and assist clients in navigating healthcare and assistance programs.
Requires a Master of Social Work (MSW) and a valid LMSW license in South Carolina. Preference is given to candidates with at least two years of healthcare experience and LISW certification.
Master of Social Work (MSW) from a CSWE-accredited program required or Bachelor of Social Work (BSN) with significant relevant experience may be considered.
Minimum two (2) years of social work experience in a healthcare, hospital, or community health setting preferred. Experience with geriatric populations, long-term care navigation, or community health access programs strongly preferred. Familiarity with Medicare, Medicaid, PACE, and South Carolina assistance programs highly desirable.
Licensed Master Social Worker (LMSW) in the state of South Carolina in good standings, required at time of hire. Licensed Independent Social Worker (LISW) preferred; supervision toward LISW available if eligible.
