The role involves planning and executing outreach events to source new referrals and building partnerships with community agencies in The Bronx. Additionally, the worker manages the program waitlist and maintains a small caseload of older adult clients.
A Bachelor's degree in social work is preferred, and candidates should have at least 3 years of experience working with older adults. Proficiency in both English and Spanish is required along with strong clinical and organizational skills.
Plan, coordinate, and execute dynamic and engaging outreach events across Community Districts 9, 10, and 11 of The Bronx. Build new partnerships with agencies and providers across a wide range of modalities, such as hospitals, rehab facilities, NYCHA developments, faith-based organizations, libraries, and council member offices, among others. Maintain, track, and be able to report effectively on all outreach activities completed, and all new partnerships built within the community. Facilitate effective public education activities, which engage new community members, as a source of new referrals to the Case Management program. Maintain and track all new referrals and intakes to ensure knowledge of which partnerships and engagements result in new cases for the program. Collaborate with the Intake Social Worker to track all new referrals and intakes which become cases assigned to social workers. Implement community engagement strategies to increase visibility of the Case Management program. Manage the waitlist for new intakes to the program, prior to assignment to social workers. Assist in tracking fulfillment of program standard requirements, related to 24 hour and 15-day deadlines, prior to intake assignment. Fulfill other programmatic needs or case management duties as assigned. Manage a small caseload of clients. QUALIFICATIONS AND EXPERIENCE: Bachelor's degree in social work preferred (LMSW desired) At least 3 years of social work experience in working with the older adult population Bilingual in English/Spanish Strong interpersonal, clinical, organizational, and presentation skills Familiarity with benefit, entitlement, and other resource applications and re-certification procedures Experience and familiarity with performing home visits and engaging older adults in their environments A passion and sensitivity for working with older adults PROFESSIONALISM AND OTHER QUALITIES: Committed to aging and disability equity, inclusion, and empowerment Culturally responsive and community-minded Detail-oriented with strong documentation and organizational skills Comfortable balancing community-based work with administrative responsibilities Self-directed and proactive, with the ability to multitask in a fast-paced program environment
This is a part-time position (25 hours per week). Work schedule includes a hybrid structure of outreach, remote admin hours, and in-office team coordination. Participation in NYC Aging trainings is required.
