Coordinate supportive services, advocacy, and case assistance for older adults to help them remain independent in their communities. Develop innovative programming, maintain meticulous case documentation, and collaborate with local agencies to achieve client goals.
Requires a Bachelor's Degree in Social Work and experience working with older adults. Candidates must be bilingual in English and Spanish with strong organizational and communication skills.
Provide advocacy, support, and general case assistance services, including intakes and referrals, to all older adult community members (age 60 and older) who need assistance or support. Collaborate with other agencies and service providers in the local community, in order to achieve client goals and build parternships. Innovate programming and workshops, based on the desires and needs of community members. Execute thorough documentation of encounters with community members in detailed and meticulous case note drafting. Be pro-active for ensuring desired outcomes are achieved, in connection with community members’ goals. Enter units within the Peerplace database for all encounters with community members, as well as other units when necessary, in the permissible timeframe. Draft and share a detailed agenda for all supervision meetings in advance of the start time for each session. Educate community members on service availability, application procedures, housing rights, and engage in advocacy when appropriate. Demonstrate the ability to exercise due diligence in follow-ups with community members, their relatives or support systems, and third party providers for desired outcomes. Develop and implement support and activity groups on a wide range of topics, with community members across the aging spectrum. Assist community members in an effort to strengthen their informal support networks with other neighborhood residents, families, and friends. Assist with cooling center duties, when necessary. Perform other duties as assigned by the program director, associate director, or department director. QUALIFICATIONS AND EXPERIENCE: At least a Bachelor’s Degree in Social Work Strong program management skills Experience working with older adults Superior interpersonal and organizational skills Superior communication skills (written and oral) Bilingual fluency (English/Spanish) Be able to lift and carry at least 10 lbs.
