The Advanced Practitioner Nurse is responsible for delivering palliative care and hospice-related health care services to patients and families. This includes documenting medical histories, collaborating with care team members, and ensuring appropriate symptom control measures are in place.
Candidates must be a Registered Nurse licensed as an Advanced Practice Nurse in Texas, with a minimum of 3 years of nursing experience, preferably at the APN level. Additional accreditation in advanced practice nursing is preferred, along with comprehensive knowledge in medicine, pharmacology, and hospice care principles.

Community Healthcare of Texas is currently recruiting for an Advanced Practitioner Nurse.
Under the direction of the Chief Medical Officer, and in accordance with CHOT policies and procedures, is responsible and accountable for collaboration with CHOT physician(s), and other care team members, in an active practice to deliver palliative care and hospice-related health care services to patients and families.
