Provide primary care medical services to vulnerable community members via the Mobile Health Center and OHRA Center. Work within a multi-disciplinary team to support the emotional, social, and medical needs of adults and children.
Must be a board-certified or eligible graduate from an accredited school with an active Oregon license, DEA license, and CPR certification. Experience with low-income families and migrant health is preferred, and Spanish proficiency is valued.

We are in search of experienced, dynamic advanced practice providers to join our Outreach team. As a primary care medical home, we serve 28,000 patients annually throughout our seven health care centers, dental clinic, and mobile health center. We are an innovative non-profit organization, with the focus on providing exceptional health care to all who have barriers to care and include integrative behavioral and dental health care in our practices.
At all our primary care sites, our multi-disciplinary teams focus on providing excellent medical care and supporting our patients' emotional and social needs, both adults and children. These teams include a mix of physicians, family nurse practitioners, physician associates, integrated behavioral health team, nurses, MAs, registration personnel, and team leads. Each team has a clinical leader, and our practice also employs a psychiatric nurse practitioner and a child and adolescent psychiatrist. Our primary care sites provide outpatient services with no inpatient or obstetrical responsibilities.
This role will work on our Mobile Health Center (various sites) and at the OHRA Center (Ashland, OR), which focuses on serving our most vulnerable community members and reaching those who are unable to gain access to our health centers. This is a unique opportunity that will make a great impact to our community. This role will require regular travel to different sites within Jackson County.
