Coordinate support services and collaborate with community partners to help clients in the Short Term Post-Hospitalization and Housing Program obtain housing within six months. Provide expert case management, develop individualized plans, and maintain detailed client records in HMIS and WellSky.
Requires a high school diploma, a valid CA driver's license, and experience working with disadvantaged populations. Preferred qualifications include education in social services or substance abuse and proficiency in HMIS data management.
Under the supervision of Shelter Director, the Program Case Worker will coordinate support services and collaborate with community partners to assist clients in the Short Term Post-Hospitalization and Housing Program obtain housing and other needed community services within 6 months.
• Ability to lift up to 25 lbs. (usually file boxes)
