Position Summary
The Vocational Specialist (QP) assists individuals served by the ACT Team in obtaining and maintaining competitive employment while supporting overall recovery, independence, and community integration. Connect individuals with transportation, benefits, and community resources

Participate in treatment planning and interdisciplinary team meetings
Provide progress updates and recommendations to the treatment team
Coordinate care with families, referral sources, and community providers 📝 Documentation & Compliance
Complete timely and accurate documentation of services provided
Maintain compliance with ACTT, Medicaid, CARF, and state regulations
Participate in quality improvement initiatives and peer record reviews 🚗 Crisis & Community Response
Provide transportation for clients when needed
Participate in rotating crisis response and emergency coverage as assigned
Assist with hospital diversion and community stabilization efforts ✅ Qualifications Required:
Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or related field
Must meet Qualified Professional (QP) status under NC regulations (10A NCAC 27G.0104)
Valid driver’s license with acceptable driving record
Ability to work in the community and transport clients as needed Preferred:
Experience in vocational rehabilitation, supported employment, ACTT, housing services, or mental health
Experience working with individuals with severe and persistent mental illness
