• Hospice Social Worker (MSW / BSW) Seasons Hospice Seeking Social Worker in Fayette County and Karnes County Some social work roles support systems.

• Seasons Hospice is seeking a compassionate Hospice Social Worker to join our interdisciplinary care team.
• In this role, you will help patients and families navigate the emotional, social, and practical challenges that come with serious illness, ensuring they receive care with dignity, compassion, and support.

• What You'll Do Assess psychosocial needs of patients and families.
• Provide counseling and emotional support during end-of-life care.
• Assist with advance directives and care planning.
• Connect families with community resources and services.
• Collaborate with nurses, physicians, and the interdisciplinary hospice team.
• Help families navigate difficult decisions with compassion and guidance.

Qualifications

MSW or BSW from an accredited social work program. At least 1 year of healthcare experience (hospice preferred). Understanding of hospice philosophy and end-of-life care.
