The Unit Based Pharmacist integrates clinical and distributive pharmacy activities to manage medications in critical care settings. Key duties include performing medication reconciliation, conducting patient assessments, and collaborating with physicians to optimize evidence-based treatment plans.
Candidates must hold a Bachelor's degree from an accredited college of Pharmacy and a Registered Pharmacist License. ACLS certification is required within six months of hire, along with proficiency in computer platforms and strong communication skills.
A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes: Validate medication reconciliation: Reconcile medications upon admission for admitted patients. Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care. Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients. Educate patients about new medications. Counsel patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization. Facilitate discharges: Identify barriers to patient medication adherence and work with the interdisciplinary team to address these issues prior to discharge. Collaborate with the doctors to optimize cost-effective and evidence based: Review and validate all orders prior to the first dose administered. Monitor patient medication profiles daily. Participate in patient care rounds. Drive improved outcomes: Prevent 30 day readmissions, increase HCAHPS scores, and improve patient safety indicators. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications: Education,
Required: Bachelor's Degree from an accredited college of Pharmacy. Possess strong interpersonal skills and work well at the team level, as well as independently. Preferred: Doctoral Degree in Pharmacy. A minimum of one (1) year Pharmacist experience in a hospital setting. Licenses and Certifications Required: Registered Pharmacist License.
