Conduct initial comprehensive assessments of hospice patients and families to determine eligibility and immediate needs. Develop initial care plans and collaborate with physicians and the core team to ensure high-quality compassionate care.
Must be a graduate of an accredited nursing school with a current State Registered Nurse License and a valid driver's license. Requires one to three years of clinical RN experience, preferably in hospice or home health settings.
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Current State Registered Nurse License Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance One to three years of experience as an RN in a clinical care setting, hospice or home health preferred.

Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Review clinical referral information and responds to requests and inquiries as appropriate Collaborate with Patient Care Manager, attending physician and hospice physician regarding the eligibility and appropriateness of client for hospice services Conduct initial meeting with the patient and family over hospice service and philosophy and obtains consents for care Provide the initial comprehensive assessment of immediate physical, psychological, social and spiritual needs of the hospice patient and family and initiates appropriate interventions Develop an initial care plan that establishes immediate goals and interventions, based on nursing assessment and collaborates with the attending HMD and core team Maintain regular communication with the Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and help with managing needs (this may include unusual or potentially problematic patient/family issues) Timely submission of all documentation Performance of other duties as required Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures indeed123 It is this Agency's policy to provide equal employment opportunities without regard to age, race, color, religion, military status, gender preference, sex, marital status, national origin or disability.
