Requires a Master's degree in Nursing and valid Illinois Nurse Practitioner licensure. Two or more years of experience as a Nurse Practitioner is preferred.

The Nurse Practitioner role involves providing high-quality, comprehensive healthcare services to a diverse patient population. The NP will work in collaborative environment with physicians and other medical support staff to ensure the delivery of accessible and compassionate are. This role includes both direct patient care and patient education, as well as supporting our community health initiatives and preventative care.
10. Attend all relevant clinical department meetings, pertinent health care facility site meetings, and any additional meetings that may arise, necessitating the provider's participation.
1. Extensive knowledge of primary care practices, including management of acute and chronic conditions, health promotion, and disease prevention.
2. Strong understanding of patient care management across diverse populations, particularly in underserved communities.
3. Ability to work effectively with a wide range of healthcare professionals in a collaborative team-based environment.
4. Strong communication skills, with the ability to educate and engage patients in their healthcare decisions.
5. Proficiency in the use of electronic health records (EHR) and other healthcare technology.
6. Ability to manage a diverse caseload while balancing patient care with administrative responsibilities.
7. Cultural competency to work with patients from a variety of racial, ethnic, and socioeconomic backgrounds.
8. Ability to educate patients on lifestyle modifications, preventive care, and self-management of chronic conditions.
9. Strong organizational and time management skills, ensuring efficient patient flow and quality care.
10. Ability to adapt to the needs of an FQHC setting, working with both underserved and vulnerable populations.
11. Strong problem-solving skills with the ability to think critically and make clinical decisions in a fast-paced environment.
1. Illinois Nurse Practitioner licensure or eligibility for licensure in the state of Illinois. 2. Master’s degree in Nursing required. 3. Two plus years of experience as a Nurse Practitioner preferred.
Light physical activity performing non-strenuous daily activities of a clinical nature. Must be able to lift up to 50 pounds.
Normal work hours for this position are Monday through Friday dayshift, unless otherwise specified by management staff. Travel to other clinical locations may be required.

Be aware and understand the expectations and responsibilities of the Patient Centered Medical Home. Understand role, complete relevant duties, and the requirement to maintain highest level of proficiency within scope, as part of this team-based approach in the patient care continuum.
