Provide community-based mental health services to individuals with serious mental illness in settings like homes and shelters. Work within a multidisciplinary team to manage a shared caseload focusing on recovery and independent living.
Candidates must be licensed or license-eligible (LMSW, LGPC, LCPC, LCSW, or LMFT). Strong communication skills and a passion for serving individuals with serious mental illness are required.

Are you a therapist who wants to do more than sit in an office all day? Join our Assertive Community Treatment (ACT) team and help individuals with serious mental illness live independently, stay out of the hospital, and build meaningful lives in their communities.
This is not traditional outpatient therapy. ACT is hands-on, team-based, and deeply impactful work—where every day is different, and your support meets clients where they are.
What You’ll Do: · Provide community-based mental health services in homes, shelters, and other real-world settings
· Work as part of a multidisciplinary team (psychiatry, nursing, case management, peer support)
· Share responsibility for a team caseload—no isolated practice
Support clients with: o Symptom management
o Crisis intervention
o Housing stability
o Substance use recovery
o Daily living skills
· You’re not alone: Shared caseload + daily team support
· Flexible, field-based work: Less back-to-back sessions, more meaningful engagement
· Real outcomes: Help reduce hospitalizations and support long-term recovery
· Creative problem-solving: No “one-size-fits-all” treatment plans
What We’re Looking For: ·Licensed (or license-eligible) (LMSW, LGPC, LCPC, LCSW, LMFT)
· Strong communication and teamwork skills
· Comfort working in community settings and adapting to changing situations
· Passion for serving individuals with serious mental illness
