• Occupational Therapist/ Home Care Case Manager Greater Cincinnati, Ohio Area Salaried Position General Summary/Responsibilities: The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences.
• When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services.
• The Occupational Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.
• The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.
• In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

• Area of study or major: Occupational Therapy Licensure/Certification/Registration  Current licensure to practice Occupational Therapy in Ohio 

• Required: 1 - 3 years of job-related experience
• Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.
• Preferred experience: One year clinical experience in acute care setting preferred Knowledge/Skills  Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.  Must be able to creatively adapt multiple environments and situations for successful outcomes.  Must be self-directed and able to work with little supervision.  Computer skills preferred.  Home care experience preferred.
