Conduct psychosocial assessments and develop comprehensive care plans for hospice patients and their families. Coordinate with interdisciplinary teams to provide emotional support, education, and resource referrals for end-of-life care.
Requires a Bachelor's or Master's degree in Social Work and valid licensure as an LCSW or LCSWA in North Carolina and LISW-CP in South Carolina. Proficiency in computer systems and experience in medical social work or hospice care is preferred.
