The Care Coordinator manages comprehensive care for individuals with intellectual and developmental disabilities by developing person-centered support plans. They act as a liaison between members, families, and healthcare providers to ensure seamless service delivery and optimal health outcomes.
Candidates must have a bachelor's degree in a social science field or a nursing license (RN/LPN) with at least two years of relevant experience with IDD populations. Proficiency in electronic health records and basic computer applications is required.
