The Case Manager monitors an outpatient caseload, developing individual program plans and assisting clients with housing, employment, and discharge planning. They coordinate with clinical teams, referral providers, and family members to ensure treatment compliance and service delivery.
A bachelor's degree in Criminal Justice, Human Services, or a related field is required. Experience in case management and working with correctional populations is preferred.
