• Job Summary: The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care.
• You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team.

Duties and Responsibilities

Recognize and report changes in patient condition to the RN supervisor Complete timely, accurate clinical documentation Support and carry out the patient’s plan of care alongside the RN Collaborate with therapists and other clinicians to ensure continuity of care

Qualifications

Current Missouri LPN license At least one (1) year of recent LPN experience (acute, long-term care, or home health preferred) Home health experience preferred At Haven, our values guide how we care for patients and how we show up for each other.
